Neck lift scars: Where they appear and how to minimize them

When it comes to facial surgery procedures, the scars left by a neck lift are the most discreet. This is due to the fact that the incisions are small and placed in an area that can easily hide them. They are not, however, completely invisible, and the final result depends not only on where they are placed, but how the skin is closed, and how well the healing period is managed.

Neck lift incision placement

A neck lift commonly requires only one or two incision sites, depending on what the surgery is supposed to accomplish. In other words, the neck lift incision pattern follows the technique and not the other way around.

The first is a small incision under the chin, hidden in the natural crease where the chin meets the neck. This is the access point used to tighten the neck muscle when vertical bands are present. When the inner edges of the platysma have separated, they are sutured back together through this incision in a corset platysmaplasty, which recreates a firm muscular sling beneath the chin. Why do neck lift scars heal differently from facelift scars?

In fact, neck lift scars follow the same basic healing sequence as any surgical scar, passing through an early red and slightly raised stage before fading and flattening over the following months. The differences lie in the tissue itself. Neck skin is thinner and more malleable than the skin in front of the ear, moving constantly with everyday activity; for example, every time a person swallows, turns their head, or looks down. These repeated movements result in low-level stress on the healing incision, making a period of reduced neck motion essential to the healing of an under-chin scar.

The muscle repair underneath adds a second healing factor. Because a platysmaplasty tightens the muscle into a new position, the tissues are under more tension than a simple skin closure would be. Tension is one of the main drivers of the widening or thickening of a scar. It is a determining factor in the way the closure is planned and supported, and impacts the neck as much as the incision placement.

How long is the neck lift recovery period?

Understanding neck lift recovery helps set realistic expectations for when scars will settle and when normal life resumes. These two factors work together in the healing process, since much of what protects a scar happens during the first few weeks.

In the initial days, a dressing supports the neck and helps control swelling. Because a neck lift is often combined with liposuction, some fluid leakage from the incisions in the first days is normal, and bandages are typically removed at the first postoperative visit, around a week after surgery. Showering is generally resumed only once the surgeon confirms the incisions can be exposed to water.

Most patients feel comfortable returning to desk work within roughly two weeks, though this can vary. Bruising fades over the first couple of weeks, while swelling under the chin and along the jaw resolves more gradually over the following months. Strenuous activity, including heavy lifting and anything that strains the neck are restricted during the early recovery period, both for safety and to protect the healing incisions from tension.

What can impact proper neck lift scar healing?

Most neck lift scars heal easily and are barely visible to the untrained eye. The most common problems usually depend on tension in the area or to the individual’s tendency toward heavier scarring. A widened scar behind the ear is the most common, occurring where the skin closure carries the most pull. When a scar is closed under tension, it can stretch as it heals, leaving a broader line than intended.

A visible or depressed scar under the chin is the neck-specific concern. Because this incision sits in a mobile, shadowed area, a scar that heals slightly indented or bound down can occasionally catch the light. Careful closure and protecting the area from strain during healing reduces the risk.

Thickened or raised scarring can also occur in patients who are naturally predisposed to it, regardless of the surgical technique used. How can you minimize neck lift scars?

Many people tend to overlook the importance of protection the neck from the sun, when in fact it is far more exposed than the skin behind the ear. Ultraviolet light on an immature scar can cause lasting darkening, so keeping the area shaded or covered for a year or more after surgery is advised.
